Eligibility & Dependent Certification Requirements

You are eligible to enroll in coverage under the plans described on this web site as long as you are a full-time, part-time, or limited-time faculty, staff or bargaining unit member.

In addition to yourself, you may also cover your eligible dependents. Dependent eligibility and documentation requirements are shown below.

Spouse Copy of legal marriage certificate (English translation) or Affidavit of Marriage
Domestic partner Affidavit of Domestic Partnership
Biological child until end of the year child turns 26 Copy of birth certificate

Verification of birth from hospital (must contain parent’s name)

Step-child and child of employee’s domestic partner until end of the year child turns 26 Copy of birth certificate or Affidavit of Domestic Partnership showing your spouse/partner’s name; and a copy of the marriage certificate showing the employee and parent’s name
Adopted child until end of the year child turns 26 Copy of proof of legal guardianship
Child for whom you are the legal guardian, including children placed for adoption until end of the year child turns 26 Copy of proof of legal guardianship
