Johns Hopkins University offers a choice of five life insurance plans.

| Plan 2 | $50,000 coverage | You pay for coverage over $10,000 |
| Plan 3 | 150% of your base salary | You pay for coverage over $10,000 |
| Plan 4 | 250% of your base salary | You pay for coverage over $10,000 |
| Plan 5 | 400% of your base salary | You pay for coverage over $10,000 |
If you are a new employee, or an employee with an eligible life event, you can elect the highest level of life insurance and you WILL NOT have to complete a Statement of Health. The cap on life insurance is $2 million.
If, as a current employee, during open enrollment you elect to move up more than one level of life insurance you will have to submit a Statement of Health. If you move up only one level you WILL NOT have to complete a Statement of Health, unless your request hits or exceeds $500K.

Bargaining Unit
Your basic life insurance benefit is equal to your base annual salary, rounded
to the next lowest $1,000. You can also purchase extra coverage equal to 100%
of your base annual salary.
